Note for applicants that have applied to Ryerson in a previous academic year: If the name on the application has changed in any way, proof of this name change (i.e. marriage certificate, notarized statement or other legal document) must be provided.

Application Forms

Use this form if you are a current Ontario secondary school student.

The external,Ontario Universities' Application Centre (OUAC) has a Base Application Fee which covers the initial three university/program choices. There are added charges for additional choices. These are noted in the external,Summary of Fees for 101 Applicants.

OUAC 105D

Use this form if you currently reside in Canada (Canadian citizens, permanent residents or those currently studying in Canada on a student permit or other visa) or you are a Canadian citizen living elsewhere (not in Canada) and you are not currently attending an Ontario secondary school in a daytime program of study.

OUAC 105F

Use this form if you currently reside outside of Canada and you are not a Canadian citizen and you are not currently attending an Ontario secondary school (in Ontario or abroad) in a daytime program of study.

Note: external,The Ontario Universities' Application Centre (OUAC) has a Base Application Fee which covers the initial three university/program choices. There are added charges depending on the type of application you use and any additional choices. These are noted in the external,Summary of Fees for 105 Applicants.

Use this form if:

  • You are a current Ryerson student
  • You are a previous Ryerson student (since fall 1984)
  • You are only applying to a part-time program at Ryerson
  • You are applying to be a Special Student
  • You are applying for re-admission or re-instatement

Exception: Applicants to the Midwifery program must apply through the Ontario Universities' Application Centre (OUAC) unless applying for readmission or reinstatement to the program, in which case a Ryerson application must be submitted.

The Ryerson Application has a fee of $85 payable via the OUAC. This fee covers, and is limited to, two program choices on the application.

Note: If you are a student in a Ryerson Undergraduate program requesting a plan change within your program, please consult your program department for appropriate procedures.

Current or previous Chang School of Continuing Education and/or Yeates School of Graduate Studies students are eligible to use the Ryerson Application.

Use this form if you applied using a Ryerson Application. Please note that if you applied using an OUAC 101 or 105 application form, changes must be made directly to the external,OUAC using their amendment process.

After your Ryerson Application has been acknowledged, use this form to:

  • Make changes to your application (e.g. application details, address, telephone, e-mail address, etc.)
  • Add or drop a program choice (maximum of two active program choices)

Supplemental Information

If you applied using an OUAC 105D, OUAC 105F or Ryerson Application Form, you are required to complete and submit a Supplementary Form for EACH program you applied to.

The following applicants are NOT required to complete the Ryerson Supplementary Form:

  • OUAC 101 applicants (current Ontario Secondary School Students)
  • Applicants for Special Student Status
  • Applicants to the Midwifery program (applicants to the Midwifery program must complete the Midwifery Supplementary Form)

Please note that if you are currently enrolled in Grade 12 secondary school studies in one or more of the following Canadian curricula, you are required to complete Section 2 of the Supplementary Form (Self-Reported Grades): Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Northwest Territories, Nova Scotia, Nunavut, Prince Edward Island, Saskatchewan, Yukon.

Here’s how to access and submit your Supplementary Form and other documents:

  1. Get your Ryerson Student Number in your applicant portal account by visiting external,choose.ryerson.ca
  2. Activate your Ryerson Online Identity. Fill in the Activation Form at www.ryerson.ca/accounts/. When asked for your Student/Applicant ID, enter your nine-digit Ryerson Student Number. Keep a record of your user name and password.
  3. 24 hours after activating your Ryerson Online Identity, visit the Document Upload Page.

Use this form if you applied to a Ryerson program and you have NOT received an admission decision (approval, denial, wait list, cancellation).

You may provide confidential information on this form and attach related documentation (e.g. medical certificates, letters of support, etc.) that you wish to be taken into consideration during our review of your application for admission. The information and documentation provided will be considered confidential and will be included in your official application to Ryerson University. The information and documentation submitted will not be provided to any other department at Ryerson, including the Academic Accommodation Support office.

We endeavour to consider all requests for special consideration which are deemed reasonable. Ryerson is committed to reviewing each situation on a case by case basis, in keeping with the principles of fairness and equity for all applicants. Due to the competitive admission and selection process, and the limited space in our programs, only those applicants selected by our admission committees will receive Offer(s) of Admission.
Ryerson University does not issue alternate Offers of Admission to applicants who are not selected for admission.

We encourage you to submit this information prior to our application deadline date for equal consideration (February 1st). Information received after the deadline date will be considered subject to space availability and competition.

This form is no longer available for use for Fall 2017 admission.

Use this form if you have been denied admission to a Ryerson undergraduate degree program and you would like to request that your application be reconsidered.

Do not use this form if you have an unsuccessful/incomplete English Proficiency test result. Visit English Language Requirements for Requests for English Language Proficiency Test Reconsideration or Exemption.

You may provide information on this form regarding extenuating circumstances and attach related documentation [e.g. medical certificate(s), updated unofficial academic transcript(s), etc.] that you wish to be taken into consideration during our second review of your application for admission. This information must be received within 10-15 business days of the date of your admission decision.

The information and documentation provided on this form will be considered confidential and will be included in your official application to Ryerson University. We endeavour to consider all requests for reconsideration which are deemed reasonable. Ryerson is committed to reviewing each situation on a case-by-case basis, in keeping with the principles of fairness and equity for all applicants. Due to the competitive admission and selection process, and the limited space in our programs, only those applicants selected by our admission committees will receive Offer(s) of Admission.

Ryerson University does not issue alternate Offers of Admission to applicants who are not selected for admission.

You will receive a response to this request for reconsideration via your Choose>Ryerson Applicant Portal, by email, or by telephone within 3-4 weeks (or sooner) of submission of this form. Ryerson will only reconsider an admission decision once, upon receipt of this form.

Here’s how to access and submit a Reconsideration of Admission Decision Form:

  1. Get your Ryerson Student Number in your external,Choose>Ryerson Applicant Portal.
  2. Activate your Ryerson Online Identity. Fill in the Activation Form at www.ryerson.ca/accounts/. When asked for your Student/Applicant ID, enter your nine-digit Ryerson Student Number. Keep a record of your username and password.
  3. 24 hours after activating your Ryerson Online Identity, visit the external,Reconsideration of Admission Decision Form.

Use this form if:

  • You applied to a Ryerson program for Fall 2017 on an Ontario Universities’ Application Centre (OUAC) 101, 105 or Ryerson Application Form
  • You have received an official Offer of Admission for Fall 2017
  • You would like to request a deferral to Fall 2018

Deferrals to spring, summer or semesters beyond one year will not be considered. Please see the Request for Deferral of Admission Form for additional information and conditions.

All applicants, including those given an early or conditional Offer of Admission, are required to successfully complete all admission requirements by June 30th (unless otherwise stated in an official Offer of Admission).
