UNDERGRADUATE PROGRAM CALENDAR 2004-2005 | ||
BACHELOR OF SCIENCE IN NURSING - PRIMARY HEALTH CARE NURSE PRACTITIONERAdministered by the School of Nursing
DEGREE: Five semesters of study for Registered Nurses beyond a diploma program in Nursing.
ADMISSION
1. Current registration by the College of Nurses or eligibility for registration in Ontario.
2. Graduation from an Ontario College of Applied Arts and Technology (CAAT) diploma program in Nursing with a minimum of “B” average overall.
3. Potential applicants from diploma programs other than a CAAT will be assessed on an individual basis.
4. Preference will be given to Ontario residents whose current experience in any setting has been continuous and demonstrates critical thinking, potential to work with clients in a collaborative, holistic, family-centred practice, or advancement of evidence-based practice.
5. Applicants must have the equivalent of two years full-time nursing practice within the past five years.
6. Other documents, such as high school transcripts, written profiles, clinical references will be required. Please request a detailed admissions package.
7. Refer to Police Check Information in the “Additional Nursing Program Requirements” section of the four year Nursing program.
ADMISSION PROCEDURE1. All applicants are required to submit the OUAC application form, personal questionnaire, two clinical references and the professional experience form included in the application package.
2. Submit to the Office of Undergraduate Admissions officially certified academic transcripts including graduation status of all nursing and other post-secondary level studies, indicating specific courses and grades.
3. Photocopy of current Ontario Certificate of Registration or proof of eligibility for same, must also be submitted to the Office of Undergraduates.
