WHEN is the risk assessment form filled out?
Well in advance of the start of funded or unfunded research, curriculum planning, special events on campus, or an activity that will be occurring off-campus involving any activity with an inherent physical, biological, chemical, radiation and/or mechanical hazard.
WHO fills out the Risk Assessment form?
Any person or group who is initiating, organizing and/or co-ordinating the particular activity.
WHY is the form needed?
To identify hazards and estimate risks to human health or the natural environment. The risk assessment form is designed to lead the principal investigator/faculty advisor or project supervisor to predetermine the necessary control measures PRIOR to the start of the research or project or purchase of equipment.
