Preceptor Nomination Forms (F2011/W2012) 

If you would like to nominate your preceptor for an award of excellence, please download the form below, complete and submit to cpo@ryerson.ca by April 6th, 2012. Awards are presented at our annual Partnership Appreciation event.

*NEW* Toronto Police Services (TPS) - Vulernable Sector Screen (VSS) change

As you know, you are required to complete and submit a vulnerable sector screen (VSS) police check as part of your practice requirements record (PRR). The City of Toronto Police Services (TPS) has undergone a change to how students submit their completed VSS forms. As of Winter 2012, students will no longer be able to submit the VSS in-person. VSS forms will still be completed at the CPO but students will need to submit their completed forms via the mail service with an attached money order or certified cheque. Please see this  hand-out from TPS for more details regarding this change.
Please note: you will no longer receive a receipt from TPS upon submission of your VSS. We strongly advise that you register your mail when submitting the VSS so that your mail can be tracked. This does not affect students who live outside of Toronto.

Mask Fit Card

Students are now responsible for arranging their own mask fit test. If your card has expired or is nearing its expiry date, please make sure to renew it as soon as possible: SMH Questionnaire and Instructions
