Forms & Applications

Many forms are in PDF format. They can be completed on screen. To download the file, right-click on the link and select ‘Save link as…’. Save the link to your desktop. Open your Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, you can download if for free at http://get.adobe.com/reader/. From the file menu of Adobe Acrobat, select ‘Open’ and navigate to the file you have just saved. Open the file.

All forms are fillable online. Complete the form, print a copy, sign the documents and submit to the attention of Student Financial Assistance via the ServiceHub, POD150 for review and processing.

Scenarios Requiring Forms Required Forms
If your own income has changed. Income Change Form
If your OSAP application was for the Fall Term only, but you now plan to take courses in the Winter term as well.
OR
If your OSAP application was re-assessed by the Student Financial Assistance Office for the Fall term only, but you now are registered for the Winter term.
OR
You have any other changes, including cancellation of your OSAP application or change in course load.

If your OSAP application has been flagged with failure to academically progress or you have been placed on probation by OSAP. Academic Probation Letter
If you are a Ryerson student taking courses at both Ryerson and another institution on a Letter of Permission. Studies at Another Institution Form
