The Access Centre
Confidentiality Agreement
All material shared with the Access Centre is held in strict confidence. Although we communicate and/or discuss your accommodation requirements and related matters with your professors/instructors, we do not share your personal information or diagnosis with anyone unless you provide written consent. The purpose of these discussions is to ensure your accommodation requirements are met, resolve problems and/or work with the University on your behalf regarding academic issues. The nature of the information shared is the general category of disability and its impact on your learning, a confirmation of your registration with the Access Centre, and accommodation requirements.
There are four additional exceptions mandated by law whereby Access Centre staff is required to release private information:
- If one indicates that they may be a danger to themself or others;
- In the case of apparent, suspected or potential child abuse;
- If one reports sexual abuse by a regulated health care professional;
- In rare instances in which the courts subpoena an individual's records.
Student records are kept in locked file cabinets at the Access Centre. Please note that all staff take an oath of confidentiality and understand the importance of protecting this information. You can revoke your consent at any time by written communication to the Access Centre.
