The Access Centre
Registration
New Registration
As soon as possible, contact the Access Centre by phone (416.979.5290) or email (accessfrontdesk@ryerson.ca), to schedule a confidential registration appointment. A Student Accommodation Facilitator will meet with you to review your disability documentation and implement appropriate academic accommodations. For your Facilitator to understand the nature and impact of your disability, please send your documentation ahead of your appointment. Your documentation is held in strict confidence. See documentation requirements below.
If you would like more information about our services please attend an Orientation Session. Sessions are available through August and September.

*Note: You need to provide this Form to be eligible to make use of your accommodations.
Registered Only for Fall 2012 Semester or Not Yet Registered:
Please complete and submit a Re-Registration Form to the AC Front Desk (or complete a hard-copy--available at the Front Desk)
*Note: It is important we receive updated documentation (as indicated by your Facilitator) for continued registration.)
Documentation Requirements
Students living with ADHD, Asperger’s or a Learning Disability:
- The Access Centre requires a copy of a psycho-educational assessment or a neuro-psychological assessment; this assessment must be recent (completed within the last 3 to 5 years, or over the age of 18).
- The assessment must be completed by a registered psychologist or psychiatrist trained to test for and diagnose the condition. The assessment must contain information about the tests administered as well as the scores obtained. Students should bring this information to our office before their first appointment.
Students living with a Medical or Mental Health Condition, or with Sensory Impairments:
- The Access Centre requires the Disability Documentation Form to be completed by your doctor, specialist, psychologist or psychiatrist who is aware of your disability and can assess its impact on your academics. A copy of this form is available at the Access Centre front desk.
- All disability documentation provided to the Access Centre must clearly state the functional limitations of the disability as related to the academic experience while at university, namely the direct correlation between the diagnosis and any accommodation(s) recommended.
- All disability documentation must be sent directly to our office from the medical professional by mail, email, or fax. If the student brings in the documentation, it must be in a sealed envelope with the medical professionals signature over the envelope seal.
