Department Hiring Committee (DHC) Membership Form

Important!

The Department Hiring Committee (DHC) membership form needs to be submitted no later than June 15 of the current year.

The DHC normally consists of five members, but it can be reduced to three or increased to seven.

For details about Department Evaluation Committee please refer to the memo "Formation of Hiring and Evaluation Committees" sent to all Chairs, Directors and Deans by the Office of the Vice Provost, Faculty Affairs on April 3, 2012.

Check the box if the Chair/Director submitted the brief report negotiated by Article 4.1.E to the Vice Provost, Faculty Affairs and the President of the RFA.

Soon after the submission, the DHC composition will become available on the DHC web page. If you need to make any change to the DHC composition, resubmit the form explaining the reasons in the Comments field.
