Applications and Related Forms

Before applying, please check which Ryerson undergraduate programs are accepting applications and review the Notice of Collection and Use of Personal Information. To view this information in video form, visit our Media page.

Note for applicants that have applied to Ryerson in a previous academic year: If the name on the application has changed in any way, proof of this name change (i.e. marriage certificate, notarized statement or other legal document) must be provided.

OUAC 101 Application

Use this form if you are a current Ontario secondary school student.


The Ontario Universities' Application Centre (OUAC) has a Base Application Fee which covers the initial three university/program choices. There are added charges for additional choices. These are noted in the Summary of Fees for 101 Applicants.

OUAC 105 D and F Applications

OUAC 105D

Use this form if you currently reside in Canada (Canadian citizens, permanent residents or those currently studying in Canada on a student permit or other visa) or you are a Canadian citizen living elsewhere (not in Canada) and you are not currently attending an Ontario secondary school in a daytime program of study.

 


OUAC 105F

Use this form if you currently reside outside of Canada and you are not a Canadian citizen and you are not currently attending an Ontario secondary school (in Ontario or abroad) in a daytime program of study.


Note:
The Ontario Universities' Application Centre (OUAC) has a Base Application Fee which covers the initial three university/program choices. There are added charges depending on the type of application you use and any additional choices. These are noted in the Summary of Fees for 105 Applicants

Ryerson Application

Use this form if:

  • You are a current Ryerson student
  • You are a previous Ryerson student (since fall 1984)
  • You are only applying to a part-time program at Ryerson
  • You are applying to be a Special Student
  • You are applying for re-admission or re-instatement


Exception:
 Applicants to the Midwifery program must apply through the Ontario Universities' Application Centre (OUAC) unless applying for readmission or reinstatement to the program, in which case a Ryerson application must be submitted.


The Ryerson Application has a fee of $85 payable via the OUAC. This fee covers, and is limited to, two program choices on the application.

Note: If you are a student in a Ryerson Undergraduate program requesting a plan change within your program, please consult your program department for appropriate procedures.

Current or previous Chang School of Continuing Education and/or Yeates School of Graduate Studies students are eligible to use the Ryerson Application.

Ryerson Change Form

Use this form if you applied using a Ryerson Application. Please note that if you applied using an OUAC 101 or 105 application form, changes must be made directly to the OUAC using their amendment process.


After your Ryerson Application has been acknowledged, use this form to:

  • Make changes to your application (e.g. application details, address, telephone, e-mail address, etc.)
  • Add or drop a program choice (maximum of two active program choices)
Ryerson Supplementary Form

If you applied using a Ryerson, OUAC 105D or OUAC 105F application form, you are required to complete and submit this Supplementary Form for EACH program you applied to. If you applied using an OUAC 101 application or you applied for Special Student Status you are not required to submit this Supplementary Form.

Self-Reported Grades

This form is required if you are currently enrolled in Grade 12 secondary school studies in one or more of the following Canadian curricula: Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Northwest Territories, Nova Scotia, Nunavut, Prince Edward Island, Saskatchewan, Yukon. This form is not to be used by current Ontario secondary school students or students who are studying in Quebec.

Special Consideration Form (OUAC 101 Applicants)

Use this form if you applied to a Ryerson program on an Ontario Universities’ Application Centre (OUAC) 101 Application Form and you have NOT received an admission decision (approval, denial, waitlist, cancellation).

You may provide confidential information on this form and attach related documentation that you wish to be taken into consideration during our review of your application for admission.

Special Consideration Form (OUAC 105/Ryerson Applicants)

Use this form if you applied to a Ryerson program on an Ontario Universities’ Application Centre (OUAC) 105 Application Form or a Ryerson Application and you have NOT received an admission decision (approval, denial, waitlist, cancellation).

You may provide confidential information on this form and attach related documentation that you wish to be taken into consideration during our review of your application for admission.

Reconsideration of Admission Decision (OUAC 101 Applicants)

Use this form if you applied to a Ryerson program on an Ontario Universities’ Application Centre (OUAC) 101 Application Form and you have been denied admission.

Do not use this form if you have an unsuccessful/incomplete English Proficiency test result. Visit English Language Requirements for Requests for English Language Proficiency Test Reconsideration or Exemption.

You may provide information on this form regarding extenuating circumstances and attach related documentation that you wish to be taken into consideration during our second review of your application for admission. This information must be received within 10-15 business days of the date of your admission decision.

Reconsideration of Admission Decision (OUAC 105/Ryerson Applicants)

Use this form if you applied to a Ryerson program for Fall 2015 on an Ontario Universities’ Application Centre (OUAC) 105 Application Form or a Ryerson Application and you have been denied admission.

Do not use this form if you have an unsuccessful/incomplete English Proficiency test result. Visit English Language Requirements for Requests for English Language Proficiency Test Reconsideration or Exemption.

Request for Deferral of Admission

Use this form if:

  • You applied to a Ryerson program for Fall 2015 on an Ontario Universities’ Application Centre (OUAC) 101, 105 or Ryerson Application Form.
  • You have received an official Offer of Admission for Fall 2015.
  • You would like to request a deferral to Fall 2016.

Deferrals to spring, summer or semesters beyond one year will not be considered. Please see the Request for Deferral of Admission Form for additional information and conditions.

Request for Summer School Extension

All applicants, including those given an early or conditional Offer of Admission, are required to successfully complete all admission requirements by June 30th (unless otherwise stated in an official Offer of Admission).
