How to Complete TRS Forms
- Use black lead pencil only
- Make dark marks that completely fill the bubble
- Erase cleanly any marks you wish to remove
- Make no stray marks on the sheet
- Sheets must not be folded, bent, stapled or soiled
Master sheet— required — Contains correct answers and the anonymous survey option.
Weight sheet — optional — Contains question weights.
System sheet — optional — Provides more detail and email results options. If omitted, percentages with guessing adjustment will be provided.
Student sheet — required — One sheet for each student. Students must bubble the '3' for this sheet to be read.
Topic sheet — optional — Assigns questions to subsets.
Anonymous Surveys - TRS sheets may be used to administer a questionnaire to students instead of an examination
One Master sheet is required for each request and it must be on the top of the stack of sheets when submitted for scanning. The Master sheet can be identified by the filled-in "0" bubble on the top line in the "For office use only" section. A single Master sheet covers all course sections writing a test. If different course sections write different tests, separate TRS requests must be submitted, each with its own Master sheet.
The Master sheet is used to record the correct answers to the examination. Answers are recorded by filling one bubble for each question. Correct answers will be "1", "2", "3", "4" or "5" (or "A", "B", "C", "D" and "E") according to the bubble that has been filled. A question may be omitted by leaving the bubble blank. A question with more than one darkened bubble on the Master sheet will be omitted from the results.
Results will not be produced if:
- the Master sheet is missing
- the Master sheet contains no answers
- the Master sheet is smudged or causes the scanner to jam
If you do not submit a System sheet, you will receive percentage results by student name and section.
The Weight sheet is used to control the contribution that a question makes to the final mark. The Weight sheet can be identified by the filled-in "1" bubble on the top line in the "For office use only" section. The Weight sheet is optional.
Weights have the values 1, 2, 3, 4 or 5. If the Weight sheet is omitted, all questions will have a weight of 1. Each question can have a different weight. If the bubble for a question has been left blank, then it will be assigned a weight of 1. To give a question a weight of 2 or more, you must darken the appropriate bubble.
There is no weight of zero — if you want to omit a question, leave its bubble blank on the Master sheet.
For example, to assign questions 1 through 5 a weight of 5, fill in the "5" bubble for questions 1 through 5 on the Weight sheet:
The System sheet is used to receive results via email, transform raw scores to a new base, and select which parts of the printout you want. The System sheet can be identified by the filled-in "2" bubble on the top line in the "For office use only" section. The System sheet is optional.
How to Find Your Short ID:
- Log in to my.ryerson.ca.
- Under the "Self Service" section, click Personal Account.
- From the "General" menu, select Personal Information.
- The my.ryerson Short ID field has your Short ID.
Take note of your Short ID and use it to fill out the TRS System sheet.
The System sheet can also be used to determine the percentage contribution of the test or exam toward the total grade. For example, if the examination is worth 50% of the final grade, you would fill in the "5" and "0" bubbles in the "Identification Number" section of the System sheet:
Note: If you would like TRS data and results sent to you by email where no printout is created, fill in the System Sheet as follows. In the For Office Use Only area, in the right section, fill in the "0" bubble.
The System sheet allows you to determine in which format you would like your results. If you do not submit a System sheet, you will receive percentage results by student name and section. The parts of the report will be printed only when you darken the appropriate bubbles in the For Office Use Only area of the system sheet. The most commonly requested results formats are Default, and Parts 1 through 7.
First to indicate this is a system sheet, fill-in the "2" bubble on the top line in the "For office use only" section of the form. To request parts one to seven of the report, fill-in on the second line of bubbles, from one to seven. For part 14, fill-in on the "4" on third line of bubbles.

The Topic sheet can be used to divide your examination questions into groups that share the same topic, chapter or theme. The Topic sheet can be identified by the filled-in "4" bubble on the top line in the "For office use only" section. The Topic sheet is optional and there can be multiple topic sheet.
