Workshop Details

On Lok Lifeways is an internationally acclaimed, fully integrated program of care for older persons with multiple health and social needs. For over 40 years, On Lok has provided a range of medical care, prescript ion drugs, home care, adult day health, transportation, and more, to a culturally diverse community of “at risk” older persons, allowing them to age at home for as long as possible, safely and cost-effectively. On Lok’s groundbreaking care model has been replicated in more than 70 PACE (Program of All-Inclusive Care for the Elderly) organizations in the U.S.

In her workshop, Grace Li will discuss:

  • On Lok’s key design dimensions: who the program supports, what it provides, how care is integrated around the needs of older persons
  • How success is measured: key performance measures at organization and individual levels
  • Ongoing challenges and opportunities
  • Lessons learned: how On Lok’s successes can inform Ontario

Grace Li (M.H.A.), Chief Operating Officer of On Lok Lifeways in San Francisco, USA joined the organization in 2001 as the Director of Program Operations. She has wide-ranging experience in the hospital, managed physician, assisted living facilities and health care consulting settings. In her role with On Lok, Grace brings expertise in orchestrating new site openings, managing the integration of software applications and she leads the planning and development of new programs, as well as overseeing the operations of On Lok Lifeways' directly-operated centers.
