If you’re looking for forms and applications related to OSAP or other financial assistance, you’ll find them here.

Many forms are in PDF format and can be completed on screen. To download the file, right-click on the link and select ‘Save link as…’. Save the link to your desktop. Open your Adobe Acrobat Reader. If you don’t have it, you can download it for free on the Adobe website New window. From the file menu of Acrobat, select ‘Open’ and navigate to the file you just saved.

Need to submit OSAP forms to complete your application? If you log into your OSAP application and click on the View documents & print/upload button, you will be directed to download, scan, and upload supporting documents for your OSAP file. The button will look like this:

We have some Ryerson-specific forms below that are not available on the OSAP website:

Form Use this form if...
Academic Progress Letter You are on academic progress for OSAP
OSAP: Studies at Another Institution You are taking the majority of your courses at Ryerson and additional courses at another institution on a Letter of Permission.
Family Breakdown Review If parental information is unavailable because of a break in contact or estrangement, a Family Breakdown Review can be considered.

OSAP Appeals and Reviews

Not getting enough OSAP funding? You might be eligible for an OSAP Review. Students may request that their OSAP eligibility be reviewed if they have a special situation which is not recognized in the standard OSAP or bursary assessment of financial need. Please review our OSAP Review website for more information.

Have extenuating circumstances not covered by these reviews? The Student Financial Assistance team is available to meet with students at the ServiceHub. Remember to bring a piece of government issued ID when you come so we can access your file.

Ontario Student Assistance Program (OSAP) 
Career Boost Program
Ontario Bursary for Students with Disabilities (BSWD) Canada Student Grant for Services and Equipment for Persons with Permanent Disabilities (CSG-PDSE): To be considered for BSWD and/or CSG-PDSE. you will need to submit a Disability Verification Form .
Schedule 2 (Interest Free Status Application for students with part-time OSAP)  Complete this form to maintain interest-free status on your previous OSAP part-time student loans. You must be registered in a part-time course load in order to maintain interest-free status through Schedule 2.
U.S. Financial Aid
