OSAP Forms and Appeals/Reviews

If you’re looking for forms related to OSAP, you’ll find them here.

Many forms are in PDF format and can be completed on screen. To download the file:

  1. Right-click on the link and select ‘Save link as…’. 
  2. Save the link to your desktop. 
  3. Open your Adobe Acrobat Reader. If you don’t have it, you can download it for free on the Adobe website New window
  4. From the file menu of Acrobat, select ‘Open’ and navigate to the file you just saved.

Forms

If you are looking to apply for OSAP, you can do that online on the OSAP website.New window

If you need to submit OSAP forms to complete your application, log in to your OSAP application and click on the View documents & print/upload button. From there, you will be directed to download, scan, and upload supporting documents for your OSAP file.

We have some Ryerson-specific forms below that are not available on the OSAP website:

Form Use this form if...
OSAP: Studies at Another Institution You are taking the majority of your courses at Ryerson and additional courses at another institution on a Letter of Permission.
Family Breakdown Review If parental information is unavailable because of a break in contact or estrangement, a Family Breakdown Review can be considered.

OSAP Appeals and Reviews

If you are not receiving enough OSAP funding, you might be eligible for an OSAP Review. Students may request that their OSAP eligibility be reviewed if they have a special situation which is not recognized in the standard OSAP assessment of financial need.

Request to Use Parent(s)' Estimated Income

Has your parent(s)’ income changed substantially since 2018? You might be able to have your OSAP based on their estimated income for 2019. Check out the Request to Use Parent(s)’ Estimated IncomeNew window for details.

Request to Use Spouse's Estimated Income

Adjust Living Allowance

Not living with your parent(s)’ even though they live within 30km of Ryerson? Living with your parent(s) and paying room and board? Married, but not living with your spouse during the study period due to location? You might be eligible for an adjustment to your living allowance. Adjust Local Travel in Study Period

Credit Check Review Form

Fail the credit check? Submit the Credit Check Review FormNew window to have the decision reviewed.

Family Breakdown Review Form

OSAP expects dependent students to provide parental information on their application. If parental information is unavailable because of a break in contact or estrangement, a Family Breakdown ReviewPDF can be considered. You must demonstrate that a serious and permanent breakdown of the family relationship has occurred.

*Note: OSAP reviews must be submitted 40 days before the end of your study period. Check out the OSAP Important Dates and Deadlines page for standard deadlines. Although we can review a number of items, there are a few situations that do not qualify for review, including:
 

  • If you've received maximum OSAP
  • Your parent(s) arbitrarily refuse to contribute to your education-related costs
  • Your citizenship status
  • Independent status criteria 
  • Deadline dates
  • Funding distribution

 

If you have extenuating circumstances not covered by these reviews, our team is available to meet with students at the ServiceHub to discuss other options, such as scholarship and bursary opportunities and on-campus jobs. Remember to bring a piece of government-issued ID when you come so we can access your file.

Ontario Bursary for Students with Disabilities (BSWD) Canada Student Grant for Services and Equipment for Persons with Permanent Disabilities (CSG-PDSE): To be considered for BSWD and/or CSG-PDSE. you will need to submit a Disability Verification Form .
Schedule 2 (Interest Free Status Application for students with part-time OSAP)  Complete this form to maintain interest-free status on your previous OSAP part-time student loans. You must be registered in a part-time course load in order to maintain interest-free status through Schedule 2.
