MSW Field Placement Forms

To promote your success and track your progress at your Toronto Metropolitan UniversityMaster of Social Work placement, you and your field instructor must complete four placement forms. If you have questions about these forms, you can contact your faculty consultant.

You must complete and submit a learning plan that identifies the major learning goals you should accomplish at your placement. This plan must meet the expectations of the graduate-level curriculum.

Your field instructors must complete midterm and final field practicum evaluations identifying the degree to which you successfully met the objectives in the learning plan. You must submit their completed evaluations to the faculty consultant. If you have questions about these forms, you can contact your faculty consultant.

You and your field instructor must jointly keep an accurate record of your completed placement hours by. The two of you can choose the record-keeping method in consultation with the faculty consultant.
