Many forms are in PDF format and can be completed on screen. To download the file:
- Right-click on the link and select ‘Save link as…’.
- Save the link to your desktop.

- From the file menu of Acrobat, select ‘Open’ and navigate to the file you just saved.
If you are looking to apply for OSAP, you can do that online on the OSAP website, external link, opens in new window.
If you need to submit OSAP forms to complete your application, log in to your OSAP application, external link, opens in new window and click on the View documents & print/upload button. From there, you will be directed to download, scan, and upload supporting documents for your OSAP file.

|PDF fileFamily Breakdown Review Form||OSAP expects dependent students to provide parental information on their application. If parental information is unavailable because of a break in contact or estrangement, a Family Breakdown Review can be considered. You must demonstrate that a serious and permanent breakdown of the family relationship has occurred.|
*Note: OSAP reviews must be submitted 40 days before the end of your study period. Check out the OSAP Important Dates and Deadlines page for standard deadlines. Although we can review a number of items, there are a few situations that do not qualify for review, including:
- If you've received maximum OSAP
- Your parent(s) arbitrarily refuse to contribute to your education-related costs
- Your citizenship status
- Independent status criteria
- Deadline dates
- Funding distribution
If you have extenuating circumstances not covered by these reviews, our team is available to meet with students at the ServiceHub to discuss other options, such as scholarship and bursary opportunities and on-campus jobs. Remember to bring a piece of government-issued ID when you come so we can access your file.

Complete this form to maintain interest-free status on your previous OSAP part-time student loans. You must be registered in a part-time course load in order to maintain interest-free status through Schedule 2.
